This savings program is designed to provide you with greater access to dental care at an affordable price, offering a discounted fee schedule for services provided here at Paoli Family Dentistry. Our In-House Dental Plan
$395 per year for the first member.
$325 per year for each additional family member.
The Premium Plan includes the following:
- Two regular dental cleanings per year
- Two dental exams per year
- One problem-focused exam per year
- All necessary X-rays (bitewings and full-mouth series)
- Two fluoride treatments per year, through age 17
- 15% off implant-related services
- 25% off all other services (excluding orthodontics, Invisalign, products and merchandise)

- No deductibles
- No claims forms
- No pre-existing condition limitations
- No waiting periods
- No preauthorization requirements
Payment Policies
- One-year membership must be paid in full. Coverage begins on the date the plan is paid
- You will be notified one month prior to plan renewal
- All treatment fees are due and payable at the time service is rendered
- Appointment times are limited and you are responsible for scheduling your visits
- Due to discounts offered in the plan, missed appointments and cancellations without 48 hours’ notice are not acceptable and will be assessed a fee based on the amount of time allotted for you (minimum charge of $35)
- This plan is only for patients without dental insurance or another discount plan
- This plan is only valid at Paoli Family Dentistry
Program Guidelines
- Patient portion of the bill is due at the time of scheduling
- This program cannot be used in conjunction with another dental plan or dental insurance
- No refunds or premiums will be issues if participant decides not to utilize their plan benefits
- This program cannot be used at any other office or with any other providers
- This program cannot be used for specialists or hospital care
- This plan cannot be used for costs of dental care covered under auto or medical insurance
- Program cannot be used for injuries covered under workers’ compensation claim
- Program cannot be used for treatment which, in the sole opinion of our dentists, lies outside the realm of their capability
- Family plan eligibility includes parents and their children under 23 years of age
- Membership is not transferrable
THIS PLAN IS NOT INSURANCE and is not intended to replace health insurance. This plan does not meet the minimum creditable coverage requirements under M.G.L.c. 111M and 956 CMR 5.00. This plan is not a Qualified Health Plan under the Affordable Care Act. This is not a Medicare prescription drug plan. The range of discounts will vary depending on the type of provider and service. The plan does not pay providers directly. Plan members must pay for all services but will receive a discount from participating providers. This plan is accepted at the dental practice where you purchased the plan. If unused, you may cancel within the first 14 days after receipt of membership materials or the effective date, whichever is later, and receive a full refund, less a nominal processing fee (nominal fee for MD residents is $5, AR and TN residents will be refunded processing fee). This plan is not available in Vermont or Washington, Utah.
